Acknowledgment
How medical care accessed through Altro Health works, and Clarity’s non-clinical role.
Clarity Wellness Coaching, LLC (“Clarity”) provides wellness education, coaching, and non-clinical support. Clarity is not a medical practice and does not provide medical care, diagnosis, treatment, prescribing, medication management, or pharmacy services.
Clients who choose to pursue medical treatment may access telehealth services separately through the Altro Health platform, where medical services are provided by independent licensed medical providers.
By choosing to access medical services through Altro Health, you understand and acknowledge that:
- Medical care is provided by independent licensed healthcare professionals and not by Clarity Wellness Coaching;
- Your medical provider is responsible for determining whether telehealth is appropriate for your care;
- Medical eligibility, diagnosis, treatment recommendations, prescribing, medication selection, and dosing are determined solely by the licensed medical provider;
- Telehealth may involve electronic communications such as secure questionnaires, messaging, telephone calls, video visits, or other technology-enabled interactions depending on the service and applicable requirements;
- Telehealth has certain limitations because the provider and patient may not be physically located in the same place;
- A provider may determine that an in-person examination, laboratory testing, additional evaluation, or referral is necessary before or during treatment;
- Technical interruptions, internet outages, or other technology limitations may occasionally affect access to telehealth services;
- Medical information submitted for clinical evaluation should be provided through the secure Altro Health or medical-provider platform rather than through Clarity email, text messages, social media, or general website forms;
- Your independent medical provider may require additional telehealth consent, patient agreements, privacy notices, or other disclosures before providing care; and
- You may ask your medical provider questions regarding the nature, risks, benefits, limitations, and alternatives to telehealth before receiving medical services.
Clarity does not supervise or control the medical judgment of independent healthcare providers and does not guarantee that any particular medication or treatment will be prescribed.
All medical-related payments are processed separately through the Altro Health platform. Clarity Wellness Coaching does not collect or process payment for medical treatment or prescription medications.
This acknowledgment does not replace any telehealth informed consent, Notice of Privacy Practices, medical consent, or patient agreement required by the applicable licensed medical provider or healthcare entity.
